I don't have my insurance card with me, can I still receive care?

If you had previously provided us your insurance information, and it has not changed since we last saw you, then you don't need to worry because we have it on file for you.

How much will my copayment be?

You co-pay is usually determined by your insurance provider and found in your Explanation of Benefits (EOB). After we run your eligibility, we would be able to give you a close estimate of how much your co-pay would be. While this amount is usually accurate, the final amount is determined by your insurance provider and you are responsible for any underpayment.
